What Is A Medical Certificate For Sick Leave?

A medical certificate for sick leave is a formal document issued by a health professional to confirm that you are unfit for work due to illness, injury, or the need to care for someone else who is sick or injured. The certificate serves as proof for your employer and ensures that you are entitled to sick leave under New Zealand law.


At Unichem, our pharmacists are authorised to issue Sick Leave Certificates for a range of health conditions, provided they are within their professional scope. If you're unable to attend work due to your illness or injury, or because a family member or dependent requires your care, a Sick Leave Certificate may be appropriate.

When Can a Pharmacist Issue a Medical Certificate for Sick Leave?

Pharmacists at Unichem can issue a Sick Leave Certificate for the following situations:


  • Personal illness or injury: If you're unable to work due to your own sickness or injury, we can assess your condition and determine whether a Sick Leave Certificate is required.

  • Caring for a dependent: If you need to take time off work to care for your spouse, partner, or someone else who depends on you for care due to illness or injury, we can issue a certificate for that as well.


However, it’s important to note that pharmacists at Life Pharmacy will only issue a medical certificate for sick leave for conditions that are within our scope of practice. If your condition requires more specialised medical attention, we may refer you to a doctor.


The Process of Getting a Certificate

Consultation

You will need to have a consultation with a pharmacist at Unichem. Ideally, this consultation should be conducted face-to-face, allowing us to thoroughly assess your condition. If you’re unable to come in person, we may offer phone or video consultations, depending on the situation.

Assessment

During the consultation, the pharmacist will evaluate your condition and determine whether you are unfit for work. We will consider factors such as the severity of your illness or injury and whether you might be able to perform light duties instead of taking full sick leave.

Issuing the Certificate

If the pharmacist determines that a medical certificate for sick leave is appropriate, they will issue the certificate. The certificate will include the dates you are unfit for work and any relevant details about your condition.


Record Keeping & Privacy


At Unichem, we take your privacy seriously. All records of consultations and Sick Leave Certificates issued will be stored securely and in compliance with privacy regulations. Your consultation will be confidential, and we will only retain necessary information related to your condition and the duration of your stay.
